[Informatics] Ethical dilemmas - who is responsible for the first death caused by a self-driving vehicle?

At least the black box will be easier to find.  I can imagine the 2026 crash investigation TV documentary, "Last moments .." With human error diminished as a cause of accidents, we may discover new causes such as failure to update a vehicle OS or the man sent his dog on a journey to bring home a newspaper etc.
