Great Case Study: British National Health Service scraps IT system after 12 Billion pounds have been spent on it.

"At a checking in appointment recently the nurse filled out a templated survey, and had to make copious notes on a scrap of paper for things that hadn't been included on the computer template. At a hospital, three different phone calls failed to get details of a forthcoming appointment sent to the correct address because it was apparently input incorrectly in four different places."
