HI all, I apologise for any confusion with outcomes and assessment sheets. I should have originals at home - unless the originals are confused too. I will attempt to retrieve these over the weekend and post them to this list. However, please note that the assessment/criteria sheets merely reflect what is in the study design and can be easily constructed following the format already given.
