We've found the Natcoll books have been fantastic - www.natcollpublishing.com. The books usually come with a CD that has the exercises (and the finished products!) on them. 

They have a couple of Photoshop books - Intro to PSP, Intro to PSP for the Web, Advanced PSP, Go Wild Photoshop and a Digital Design CD.
